INFORMED CONSENT INNER BALANCE 360

1. PURPOSE OF CONSENT

This Informed Consent governs the voluntary participation of the user and/or employee in the questionnaires, assessments, and analyses provided by INNER BALANCE 360 (“IB360”), operated by INNER BALANCE 360 LLC.

By accepting this consent, the user acknowledges understanding the nature of the service and authorizes the processing of their data as described herein.

2. NATURE OF THE SERVICE

IB360 is a diagnostic and strategic analysis system based on structured questionnaires and automated processing of responses, intended to generate informational, reflective, and strategic reports.

The user expressly acknowledges that IB360 does not constitute and does not replace medical, psychological, psychiatric, therapeutic, legal, or financial advice.

3. USE OF INFORMATION

By accepting this consent, the user and/or employee understands and authorizes that:

■ Their responses may be processed to generate their individual report.

■ In an aggregated and anonymized manner, the results may also be processed by the contracting organization or employer, in the case of corporate assessments, for purposes of global analysis, organizational diagnostics, and strategic improvement.

The user understands that the contracting organization will not have access to individual raw responses, but only to aggregated and anonymized results.

4. VOLUNTARY PARTICIPATION AND RESPONSIBILITY

Participation in IB360 is voluntary. The user may choose not to participate or may discontinue participation at any time prior to completing the questionnaire.

The user assumes full responsibility for any decisions, actions, or omissions taken based on the reports or results generated.

5. CONFIDENTIALITY AND DATA PROTECTION

All data provided will be processed in accordance with the INNER BALANCE 360 Privacy Policy, and reasonable measures will be implemented to protect the confidentiality and security of the information.
